The Lady Got Implant Screw in Mouth
The patient presented to my clinic all the way from USA to get her new tooth which was missing congenitally. That means the tooth never erupted in the mouth as it was never developed. As the tooth was missing, there was no space left because adjacent teeth drifted and took the place resulting in mal alignment of all the teeth. Therefore, patient got orthodontic treatment first to regain the space so that the space can be achieved to later place an artificial tooth. The braces treatment she got from United States of America.
Now, patient came to me for the replacement of the tooth that was never in the mouth. It is usually difficult to place an implant screw where there was no tooth before as the bone is not optimized to hold the tooth or tooth alike.
This image was shot when the patient was presented to us at the Green Dentistry & Implant Centre. You can see the gap which is esthetically very unpleasant, and this was the main concern of the patient, to get an aesthetic tooth fixed into the mouth.
At the very first, we took the history of the patient and prescribed a 3D CBCT scan to get an orientation of width, height and quality of the bone where we are willing to place an implant. The patient was very quick to get the scan which was a great gesture that shows she was willing and truly motivated for the treatment. So, I got the scan and did digital implant planning which is basically the planning to select the implant screw with optimum width, height and type to insert in a proper angulation. The CBCT planning is shown in the picture below.
I always do plan my implant surgery using digital implant planning with 3D CBCT scan. It makes the procedure easier and predictable. It is also convenient for the patient as well that one virtually experiences the results of his/her surgery before actually going through the procedure, building confidence and lessening the fear of the procedure.
The CBCT planning was shared with the patient and some necessary questions were answered to build confidence and get rid of patient anxiety. the patient re assurance is very necessary in these kinds of surgeries.
On the day of surgery, prophylactic medication was administered orally, and patient was seated at the dental chair, scrubbed. Local anesthesia was administered to make the surgical area numb. During complete procedure, the patient was fully conscious and responsive.
The surgery started by marking the incision with scalpel to expose the bone. The bone was drilled to make the osteotomy preparation for the implant screw placement. Multiple x-rays were taken to confirm the direction and angulation of Implant being placed. Because the bone space was very less, we had to place a very narrow dental implant.
The Closure was done with 4/0 Vicryl Suture and patient went to home happily. The 5-day medication was prescribed to avoid any trouble in the future. Patient was recalled after 24 hours to assess the post operative wound healing. The patient reported no swelling or pain but a little discomfort that is usually after any surgery or procedure.
The sutures will be removed after 6 days of the procedure. After the osseointegration takes place after 3-to-4-month period, the patient will be reevaluated and the crown will be attached to this screw.
